5. Superannuation details – member number etc.
6. First Aid Certificate
7. National Police Clearance – valid for less than 6 months
8. Influenza Vaccination 2022- (proof of vaccination)
9. Proof of residency or proof of working rights

Additional information for Nurses and Allied Health:- Valid registration with Australian Health Practitioner Regulation Agency (AHPRA)
